At Concierge Physical Therapy, we believe you should know exactly what to expect before your first visit. Below you’ll find our rates, the insurance plans we work with, and how we handle billing — no surprises, no fine print.
Cash Pay Rates
No insurance? No problem. Our cash pay rates are straightforward and all-inclusive — you get the Therapist’s full attention for the entire session.

Payment Notice: All copays and co-insurance amounts are due at the time of service. Please bring your insurance card and a valid photo ID to your first appointment.
OUT-OF-NETWORK INSURANCE
Out-of-Network Insurance
We welcome patients with out-of-network insurance benefits. Here’s how it works:
• Payment is collected at our cash rate at the time of service.
• We will provide you with a detailed superbill after your visit.
• You submit the superbill directly to your insurer to request reimbursement through your out-of-network benefits.
• Many patients are reimbursed for a significant portion of their costs — check your plan’s out-of-network benefits before your visit.
We currently see patients with the following plans on an out-of-network basis:

CHRISTIAN HEALTH SHARE PLANS
Christian Health Share Plans
We proudly work with patients enrolled in Christian health sharing ministries, including Medishare and similar programs.
• Payment is collected at our cash rate at the time of service.
• We will provide a detailed superbill for you to submit to your health share ministry.
• Christian health share plans like Medishare typically reimburse members in full for eligible physical therapy services.
We understand that health share plans work differently than traditional insurance. We’re happy to walk you through the process and answer any questions before your visit.
